A reminder from the treasurer's office for all returning members: All invoice balances must be paid in full before a swimmer may resume participation in the 2010-11 swim season. Also please print the new 2010-11 Liability waiver form from the Financial section of this website and forward to the PO BOX. Thanks for your immediate attention to the above matters.

Printable PDF 2010-2011 Financial Obligation & Liability Waiver Form and 2010-2011 Financial Information. New and returning members must sign and return the Financial Obligation & Liability Waiver form to P.O. Box 45321, Omaha, NE 68145-0321 prior to your child participating this fall. New swimmers will also need to submit the 2010-11 USA Swimming Registration Form.
New Swimmer forms:
*2010-2011 Financial Obligation & Liability Waiver Form - bring to tryout
*2010-2011 USA Swimming Athlete Registration - bring to tryout with $53.00 payable to MAC. This amount is then forwarded to USA Swimming. Will be returned if not joining.
*Copy of state issued birth certificate, military ID, or passport. - Must have for tryout!
Returning Swimmer Forms:
*2010-2011 Financial Obligation & Liability Waiver Form - return to PO Box 45321, Omaha NE 68145-0321 prior to resuming practice in the Fall. If not on file by September 30, 2010 swimmer will not be eligible to participate.
